The Robert Jones Bandage

The Robert Jones bandage is a common external splint applied to a limb for the temporary support of a fracture before surgical intervention can occur. It is used to treat many canine and feline limb injuries (e.g., tibial fractures, severe limb lacerations). It promotes healing by immobilizing the injured area, thereby limiting swelling and providing protection from secondary trauma. Compared with other padded bandages, the Robert-Jones bandage offers limb stability, tissue fluid absorption, and protection from trauma. Generally, most of the compression is lost after several hours to days as the cotton loosens. For definitive support of a fracture the bandage should never be on longer than a few days without reinforcement using rigid splint material.

Step 1: Using white bandage tape, place stirrups on the distal 1/3rd of the limb overlapping the toes and extending approximately an equal length from the end of the leg. Be sure to tab the ends for easy separation later on.

Step 2: Using a full pound of roll cotton wrap the leg moving proximally from the toes (tearing the cotton in half down the middle may aid in application). Overlap the cotton 50% as you wrap being sure to leave some overlapping the toes. This layer is wrapped lightly with minimal compression creating a uniform thickness the entire length of the limb. Note - half to two pounds of cotton are typically used for the bandage depending on the size of the animal.

Lower ankle or below tarsal braces are used to correct symptoms such as pain and limping due to missing toes and knuckling. Generally they support or immobilize the joint for injuries to the paw, tarsal and metatarsal areas. Prosthetics or Limb Replacement

Prosthetic limbs can be a good solution when body weight or activity level necessitates the need for additional support and increased function. A prosthesis allows normal weight-sharing on all legs which helps to eliminate overuse syndrome on the sound side of the shoulder, hip, or spine. To allow for adequate suspension of the device, a minimum of two to three inches of residual limb below the elbow or knee is needed.     Wrist braces are used to correct symptoms such as limping, inability to bear weight and collapsing or hyper-extension at the wrist joint. Conditions such as Carpal Collapse, Knuckling or Paralysis, Joint Instability, Hyper-Extension, Arthrodesis, and Severe Strains can be addressed with a wrist or carpal brace. They are commonly applied to support or immobilize the joint for injuries to the paw, carpal and metacarpal areas. Wrist or carpal braces are usually used long-term for nerve damage or other chronic conditions but can also be used on a short-term basis for pre- or post-surgical applications. Based on the diagnosis, they can include various types of flexible or adjustable joints in order to allow or restrict motion at the antebraciocarpal or carpometacarpal joint axis.

    These braces can be fabricated to include a front (anterior) and rear (posterior) shell (known as a "clamshell" design) for complete immobilization for a fracture or protection and support for an osteosarcoma, for example. This design allows for frequent cleaning and inspection of the affected area, a benefit not easily achievable with serial splinting and casting or bandaging.

    Care of Open Wounds in Dogs

    My dog has a large open wound and my veterinarian recommended leaving it open to heal rather than repairing it with sutures. Why is this?

    Wounds vary according to:

    1. Cause - puncture wounds, abrasions and lacerations, etc.

    2. Location - paws, abdomen, eyes, etc.

    3. Level of contamination - clean (surgical) wounds, contaminated (infected), etc.

    Your veterinarian has considered all of these factors when deciding the best way to treat a specific wound. Sometimes the location or the amount of skin loss prevents surgical closure or bandaging (wounds on the face or high up on the leg). Sometimes, puncture wounds or other trauma force bacteria deep into the tissues. A contaminated wound that is more than a few hours old should never be closed without surgical debridement (removal of all the contaminated or dead tissue), and in some cases this may result in more permanent damage than treating the wound medically and leaving it open to heal.

    What will be done for my dog's wound?

    Abscesses (collection of pus that has accumulated in a cavity) may be lanced and cleaned under anesthesia and then left open in order to allow proper drainage and prevent the further build up of infection.

    Wherever possible a wound will be closed and sutured in order to speed healing. However, if there is gross contamination or deep infection present, the wound will be left open for topical treatment and to ensure drainage.

    Your veterinarian may need to anesthetize your dog to remove foreign material and dead tissue from the wound. If the wound cannot be surgically closed, your veterinarian will apply a protective bandage if this is possible. Your dog will receive oral or injectable antibiotics and pain medications as appropriate. In almost all cases, your dog will be sent home with a prescription of antibiotics for you to administer.

    How will I need to care for this open wound?

    When your pet was discharged from the hospital, you received specific instructions regarding your pet's home care. Some general care guidelines include:

    • In order to help the healing process, it is important that the wound and surrounding area is gently cleaned to remove any crusty or sticky debris. This will keep the wound edges clean, reduce the potential for re-infection, and allow new healthy tissue to develop.
    • Administer all of the medications as prescribed. Do not discontinue antibiotics for any reason unless you have been specifically instructed to do so by your veterinarian.
    • Your dog should not be allowed to lick or chew the open wound. Many dogs will require a protective collar (Elizabethan or E-collar) to prevent them from injuring the site.
    • It is important to prevent the skin from healing over the wound too quickly. This is particularly important with abscesses that have been lanced and drained surgically. If the wound closes prematurely, the risk of recurrence increases.

    How do I prevent the wound from closing too early?

    When cleaning the wound, gently massage the surrounding skin to open the wound and promote drainage. You may note some discharge or bleeding when you do this. Note whether it appears to be infected (a thick or colored discharge) or if it is a clear thin fluid. You should remove or allow either type of discharge to drain away. If the discharge continues to be bloody, green or yellow for several consecutive days, contact your veterinarian for instructions.

    What should I clean the wound with?

    Warm tap water is recommended for cleaning most wounds. Warm saline (salt solution) may also be used. This may be made by adding approximately one level teaspoonful (5 mls) of salt (or Epsom salts) to two cups (500 mls) of water. In some cases, your veterinarian may recommend using a dilute cleansing solution of chlorhexidine, a surgical scrub soap, or an iodine solution to help remove debris.

Some of these products are toxic if taken internally, while others can actually delay healing.

    Can I not use mild antiseptics?

    Your veterinarian may prescribe antibiotics or a suitable antibiotic cream to apply to the wound. Since animals tend to lick and chew at wounds, inadvertently swallowing medications that are applied topically, disinfectants and antiseptics intended for human use are rarely suitable for use in animals.

    What should I do if my dog tries to lick the wound?

    Many people have the mistaken belief that dog's saliva is somehow antiseptic. This is far from the truth, especially when one considers how dogs groom themselves or greet each other! A dog will instinctively lick at a wound, but this can seriously delay healing. Therefore, you must prevent this by any means possible. Elizabethan collars are the most commonly used protective device. Other options, depending on the location of the wound, include covering the wound with a bandage, a stockinette, a dog coat or a t-shirt.

    What about pain medications?

    In many cases, your veterinarian will prescribe anti-inflammatory or analgesic medication to relieve pain and discomfort. However, anti-inflammatory medications can delay healing. Once a wound has begun to heal, it is no longer as painful. Your veterinarian will assess the individual situation and determine the most appropriate medications for your dog.

    When a cat bites, its sharp canine teeth easily puncture the skin, leaving small, but deep, wounds in the skin. These punctures rapidly seal over, trapping bacteria from the cat's mouth under the skin of the victim, where they can readily multiply.

    Corticosteroids (commonly referred to as steroids or cortisone) are a class of steroid hormones that are produced in the adrenal glands. Corticosteroids are involved in a wide range of activities in the body, including the stress response, immune system response and control of inflammation, nutrient metabolism, and maintenance of blood electrolyte levels.

    An Elizabethan collar (also known as an E-collar or a buster collar) is a plastic hood or cone that helps protect injuries or wounds from further damage. These collars prevent the cat from licking or chewing at an injury on its body, or from scratching or pawing at its face or head.

    Konsten att välja rätt hundskor

    Fördelen är att ett bandage får plats i sockan och att sulan skyddar tassen och håller tass och bandage torrt. Jag rekommenderar inte dessa skor för dig som letar efter skor som ska användas mer permanent. De kommer förr eller senare trilla av när hunden springer i skogen och sulan är rätt tunn, så den håller inte hur länge som helst.

    Är ditt största problem vägsalt ska du se till att skölja av tassarna och smörja in dem med en skyddande salva om du inte använder hundskor. En del salvor fungerar även förebyggande så att du kan smörja på innan ni går ut. Om det är vinter är det viktigt att du använder en salva som inte innehåller vatten - för då kan den frysa och på så vis ge köldskador. Den bästa salvan här är enligt mig Pure Lanolin som även är vårdande men även denna salva skyddar tassarna på vintern: EBI Vital Concept Protect Plus tassalva.     Ffa att det numer finns mer att välja på. Jag har bara haft behov ordentligt en gång, när Brandy opererat bort s.k. boxerböld och de skorna satt väl "sådär".

    (Och så kan jag inte låta bli att tänka på när jag läste, för läääänge sedan, om en lite pappilon som blivit godkänd som räddningshund. Försvaret kliade skallarna och klurade på hur skor skulle anförskaffas då :) )

Jag behöver använda skor på min Borderterrier under vintern då han nästan direkt börjar halta och efter ett par 100m lägger sig hjälplös på rygg när temperaturen sjunker under -10.Har ett par skor av varianten billigare, vilka fungerar - men inte sitter på särskilt bra.

    Jag skulle rekommendera Neo Paws om du vill ha skor som sitter bra, de är vattenavstötande och håller vätan borta förvånansvärt bra. Det längre skaftet gör också att det inte rinner in vatten i skon. Men Globus orangefärgade skor är ett bra budgetalternativ. Inte riktigt lika vattentäta och något kortare i skaftet. Det är dock fortfarande en jämförelsevis bra sko med tanke på priset. Problemet har du redan flaggat för då fram och baktassar ofta ä olika stora. De som köpt dessa skor har därför många gånger köpt ett set i 2 olika storlekar. De har då haft det anda paret som reserv om skorna skulle bli blöta eller om den skulle råkat tappa en sko. Även om det är bra skor så kan man ändå tappa dem. Det är därför viktigt att spänna åt hårt. Så länge hunden är i rörelse behöver man inte vara orolig för att de sitter hår. Däremot i vila så är det inte bra med hårt spända skor. Då kan man ta av dem om det är en längre paus, eller spänna upp dem lite. Vilka skor som är bra beror också på hur hundens tass ser ut. Vi har fått in en en ny kortare modell från Hurtta som inte ligger i nätbutiken ännu som vi håller på att testa. Likaså är skorna från EQDog vettiga om man vill ha den kortare varianten.     Jo det stämmer. Jag hade valt en kortare modell. Det har kommit in flera nya modeller till butiken sedan detta inlägg skrevs. Jag skulle rekommenderas Hurttas nya skor. Gå till hundskor på www.csigora.com så hittar de dem!

    Min Västgötaspets är 12 år och har ont/artros i en tå fram och verkar ha svårt att gå på grus. Samtidigt tror jag inte att han är väldigt förtjust att gå i skor. Går det att rekommendera ngt utifrån detta ? Hälsning Susanne
